Form 1040 is the US individual income tax return. There are several versions of Form 1040. The "short" form is the 1040EZ, the income tax return for single and joint filers with no dependents. The "medium length" form is the 1040A, for individuals with income less than $100,000 and who satisfy several other requirements. Form 1040 (with no suffixes) refers to the full length individual income tax return.
